- Hearing Aid Services - 1 hearing aid per ear every 36
months with $5,000 maximum for both ears.
- Orthotics - Medically necessary orthotics limited to 1) foot orthotics, orthopedic shoes, footwear or support items used for a systemic illness affecting
HMO
Hearing aids and orthotics continue to be covered under the HMO.
