PPO subscribers only: When you have paid a non-contracted provider for services, a super bill (detailed paid receipt) should be submitted to the GGUSD insurance office. Allow up to 8 weeks to receive an Explanation of Benefits including a check for any reimbursement due you.

MEDICAL HMO: Anthem Blue Cross
Use the above link to locate a provider who participates in the Anthem Select HMO network (PCP ID: 3 or 6 digit alphanumeric). You will need a referral from your Primary Care Physician to see most Specialists. To change your PCP, *contact Anthem; allow 6 weeks processing time. *Telephone: 833-913-2236.

DENTAL SELF-INSURED PPO: Guardian
Guardian is the administrator for the GGUSD self-insured dental plan using the Guardian DentalGuard Preferred PPO network; use the above link to see an online listing of network-contracted dental providers / order cards / view EOBs... Cards issued showing only the employee’s name; valid for all enrolled family members. Call Guardian for assistance: 800-541-7846.
DENTAL HMO: United Concordia
Use the above link to find a provider in the DHMO Concordia Plus network. To change your assigned General Dentist (Provider ID: 9 digit numeric), *call United Concordia; allow 6 weeks processing time. You will need a referral from your General Dentist to see a Specialist. Telephone: *866-357-3304.
Vision Service Plan
VSP is the vision plan for all of our members. Use the above link to find a Signature Network provider in your area. Register as a user and log-in at the above link to view personalized benefits information. VSP does not issue cards as they are not needed; VSP providers work directly with VSP for you.
