Dear GGUSD Employee,

Under the Affordable Care Act, employers and insurance carriers have certain guidelines and requirements they must follow. This year all full time eligible employees may receive two new IRS forms which can be used to help you complete your 2017 Tax Return. These forms provide proof of health insurance enrollment status for you in 2017. GGUSD will send Form 1095-C to you via US mail. Form 1095-B will also be sent from the insurance carrier to all employees who are enrolled in the Anthem Blue Cross HMO plan.
